Job Details
- As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
- Patient care is delivered within organizational philosophy, policy and standards of community health practice.
- Perform admission processes and case management responsibilities in accordance with MGB Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Job Requirements
- Must have 1 year Certified Home Care experience.
- Must have 1 year Case Management Experience.
- Ability to travel to cases as assigned.
Schedule Information
- Weekend rotations are once every 6 weeks.
Additional Details
- MGB Badge Photo required for onboarding: color photo taken in past 6 months, no selfies, no filters, plain background, symmetrical headshot, shoulders up, look straight, no eyeglasses.
- MGB MVR Check required within 45 days of start for all home care positions; any hits must be reviewed and approved by MGB.
- License/Certificate Verification required for submission; primary source verification of professional license and/or certification within 30 days of start date and updated upon expiration; MA Conditional Licenses not accepted; both NURSYS and BON required for nursing licenses.
- Fit Test (N95) required within 365 days to start date and updated annually; no fit testing can be done onsite at any location except Martha's Vineyard & Nantucket if traveler extends beyond original assignment.
- MGB Nurse Aide Registry check required within 30 days of start as mandated by MA DPH for all employees.
- Documentation of MMR vaccination or proof of immunity required.
- Influenza Vaccine required during flu season (Sept 15 - April 30); no exemptions or waivers considered.
- TB Test documentation required within 3 months of start date and updated if >6 month break; can be skin test or blood test (QuantiFERON or T-spot).
- Health Statement signed by MD/DO/NP/PA required within 365 days of start date and updated if >6 month break.
- MGB Criminal Background Check required within 45 days prior to start date and updated if >6 month break; includes county and international searches if applicable.
- MGB Massachusetts CORI required within 30 days prior to start date and updated if >6 month break.
- MGB Resume required for submission including past 7 years of work experience with city and state.
- MGB Skills Checklist required for submission.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions upon acceptance.
- MGB Education Verification verified through background check company.
- MGB SSN Trace must be run with criminal check.
- MGB National Sex Offender Check required within 30 days of start and updated annually.
- MGB Exclusion Checks required within 30 days of start and annually; includes OIG, GSA, MassHealth & NH Medicaid exclusion lists.
