Position Summary:

The Travel Case Manager RN is responsible for managing the clinical and financial outcomes of patient care. The role includes utilization review, discharge planning, resource coordination, and treatment plan oversight in a fast-paced LTAC environment. The Case Manager collaborates with the interdisciplinary team to support safe, timely discharges and ensure the highest quality of care.

Key Responsibilities:
  • Conducts utilization review and monitors resource management.

  • Collaborates with the clinical team and physicians to develop and manage treatment and discharge plans.

  • Identifies and helps resolve patient and family barriers to care.

  • Assures compliance with insurance requirements and regulatory standards.

  • Manages medical necessity reviews and documentation for all admissions and continued stays.

  • Educates patients and families about post-acute care options and available community resources.

  • Maintains and submits accurate medical records and case notes per policy and state guidelines.

  • Supports fiscal responsibility by ensuring appropriate services are billed.

  • Reports major changes, conditions, or barriers to the Director of Case Management or leadership.

  • Participates in interdisciplinary team meetings and case conferences.

Preferred Qualifications:
  • 2+ years of case management experience in an acute or LTAC setting

  • Familiarity with Medicare/Medicaid guidelines and commercial insurance

  • Experience with discharge planning and interdisciplinary care coordination

  • Strong communication, documentation, and critical thinking skills
