MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

\n Licensed Baccalaureate Social Worker (LBSW) – Adult Day Health \n

Location: Columbia / Owings Mills, MD
Schedule: Monday–Friday | Day Shift | Full-Time & Part-Time Available
Salary: $55,000 - $60,000 year + Mileage Reimbursement + Sign-On Bonus  of $5,000

We’re seeking a dedicated and compassionate  Social Worker to join our team! In this rewarding role, you’ll provide evaluation, counseling, and support to clients within our Adult Day Health Program — helping them achieve greater independence and improved quality of life in their home and community.

\n \n What You’ll Do \n

    Conduct comprehensive evaluations, gathering data through records, interviews, and collaboration with team members.

    \n
  • \n
  • \n

    Develop and implement individualized treatment plans in coordination with physicians and interdisciplinary teams.

    \n
  • \n
  • \n

    Continuously assess progress, updating goals and modifying plans as needed.

    \n
  • \n
  • \n

    Provide  one-on-one counseling to clients and/or family members in the home or community setting.

    \n
  • \n
  • \n

    Prepare accurate and timely documentation, including evaluations, progress reports, and discharge summaries.

    \n
  • \n
  • \n

    Participate in team meetings to review client status and support goal setting.

    \n
  • \n
  • \n

    Submit billing and documentation promptly.

    \n
  • \n
  • \n

    Communicate effectively with case managers and team members.

    \n
  • \n
  • \n

    Perform related duties as assigned to support client care and program success.

    \n

\n \n Qualifications \n
    \n
  • \n

    Requires a Bachelor's Degree in Social Work from an accredited program.

    \n
  • \n
  • \n

    Active Maryland Social Work License required.

    \n
  • \n
  • \n

    Reliable transportation to travel between centers.

    \n
  • \n
  • \n

    Strong communication, empathy, and organizational skills.

    \n
