WE ARE LOOKING TO FILL 1 POSITION 10 AM-6 PM M-F

Job Description: Provide direct support to consumers by assisting individuals with an intellectual disability or autism with direction, supervision, medication management, monitoring, meal preparation and assistance with eating if necessary. Assist with activities of daily living while promoting independence in accordance with guidelines listed in the Individual Plan and state regulations. Practice good housekeeping skills to ensure that the Home is maintained in a clean, orderly, and safe manner.

DUTIES AND RESPONSIBILITIES:

• Provide assistance as identified in the Individual Plan in order to maintain good health, hygiene, and self-esteem.
• Utilize skills acquired during orientation and in-service training to becoming familiar with the Individual’s Support Plan in order to provide essential care and emotional support.
• Help the Individual achieve established goals to promote independence and quality of life.
• Monitor individuals’ environment for safety and ensure the Home maintains cleanliness and order.
• Contribute accurate reporting of changes in the Individual’s status through documentation and verbal communication to ensure continuity of care.
• Assist with admissions, discharges and transfers of Individuals to ensure smooth transitions.
• Accompany Individuals to and from various appointments and activities when necessary.
• Promote Abundant Living at Peterson’s quality of standards.

QUALIFICATIONS:

• High School diploma or G.E.D equivalent.
• Active driver’s license.
• Must be able to successfully pass a Pennsylvania background check.
• Ability to read, write and follow oral and/or written instructions.
• Ability to lift/assist Individuals into and out of beds, chairs and the bathroom.
• Ability to work independently and problem solve effectively.

LANGUAGE SKILLS:

Ability to read, analyze, and interpret general information in the English language.

• Practices procedures for infection control.
• Practices good safety awareness and procedures; proactively addresses safety concerns or brings them to the attention of management for appropriate action.
• Promptly reports unsafe conditions and equipment.
• Keeps facility clean, neat, sanitary and safe.
• Promptly reports missing equipment/personal possession of Individuals.
• Provide accurate reporting of changes in individual’s status through written and verbal communication.
• Maintain open, effective and consistent communications and relates well to all Individuals, co-workers, and visitors.

• Considerate and patient in dealing with Individuals, staff, families, volunteers and other quests.
