Job Description
The Case Manager plays a vital role in providing case management and remote
monitoring for a caseload of patients with chronic health conditions. Essential duties and
responsibilities include:
• Follow-up on clinical assessments and care plans to facilitate with applicable
goals and interventions of the client
• Ensure the patient's remote monitoring is being measured. Address and escalate
any concerning measurements with the medical provider
• Advocate for and support the client while engaging with health care providers on
the patient's behalf with the goal of reducing emergency room visits and/ or
hospitalizations
• Provide clinical support by providing consultation, education information around
conditions and interventions in order to maintain focus on outcomes and best
practices
• Submit progress notes and documents, records, and other related reports in an
organized, timely and accurate manner as per policy and procedure
• Provide outreach and engagement of the patient to ensure all services are
provided
• Provide crisis intervention and follow-up
• Perform other related duties as assigned
SKILLS AND/OR EXPERIENCE REQUIRED
• Bachelors OR at least 2+ years experience in related human services providing
direct services to patients diagnosed with chronic health conditions or other
disabilities OR 2+ years of experience as a medical assistant or related position
• Bilingual language a plus: Cantonese or Mandarin & English
• Effective interpersonal, communication and teamwork skills required
• Experience with harm reduction and motivational interviewing techniques
strongly preferred
• Work Location: One location - Brooklyn, NY Full-Time In-Person-There is an opportunity for work from home once training is complete

$40-$45 per hour
